Employee Benefits

LMCH offers an exceptional benefits package to employees who work 24 hours or more a week. Our great benefit package includes:

PTO - Calculated at .08077 hours per hour worked. PTO days include holidays, vacation and personal days.

Life Insurance - Term life equal to one times annual compensation, minimum of $20,000, with premium paid by the hospital. Employee has the option to increase the benefit of the policy another one times annual compensation by paying the additional cost.

Long Term Disability - Covers 60% of weekly compensation after being disabled ninety days. Cost of this coverage is paid by the hospital.

Short Term Disability - Covers 60% of weekly compensation after being disabled fifteen days. This optional disability policy is paid by the employee.

Medical Insurance - The health plan is a self funded health plan with a PPO and third party administrator. There are three tiers and two deductible options available. Tiers include 90/10, 80/20 or 70/30 options depending on the provider used. Deductible options include $1000 and $2500.

Prescription Drug Coverage - This coverage has a $20 or 20% co-pay for formulary or non-formulary drugs purchased in the hospital pharmacy. Cost of this coverage is included in the cost of the health plan and covers only drugs purchased through the hospital pharmacy.

Dental Insurance - The dental plan is a self funded plan with a third party administrator. Preventative and basic procedures start at 70% and increase 10% at the beginning of each calendar year. Dental insurance can be taken separate from the health plan with a separate cost.

Vision Insurance - The vision plan is an insured plan and included in the cost of the dental plan with the same elections available. Employees can receive one free vision examination with a VSP Provider every 12 months.

Flexible Spending Accounts - Health care and dependent care spending accounts are available and administered through a third party.

401a and 457b - Several options are available for putting before tax contributions aside for retirement. The hospital matches up to 4.5% of gross pay after 1 year of service and 1,000 hours per year worked.
